Admitting/ Reception

Reception hours are from 7:00 am to 7:00 pm, Monday through Friday, and from 9:00 am to 5:00 pm Saturday and Sunday.

The Admitting Office is located in the front lobby. You will be asked to provide the following information:

• A valid OHIP card.
• Details of any other insurance (ie, semi-private, private room coverage).
• Name of relative or next-of-kin who can be reached conveniently.
• Patients admitted because of Worker's Compensation will require a claim number and the employer's name and address.

Where possible, your requests for semi-private or private accommodation will be met as availability permits. An additional charge for these types of accommodation is your responsibility. The hospital will submit the claims to your insurance, however, the responsibility for payment remains that of the patient. In some instances, pre-payment may be required.

Consents

You or a member of your family will be asked to complete and sign a consent form for admission and possibly a consent form for a specific procedure or treatment.

The procedure to be performed will have been explained to you by your doctor. Before signing the consent, it is important that you understand what is to take place and what to expect as a result of the procedure.

If there is anything you do not understand, please ask questions. Remember, it is your right to be fully informed.
