Application for Privileges

To obtain privileges at the MICs Hospitals, physicians must provide the following:

• Completed Application for Appointment to Medical Staff   (Application form)
• Signed CPSO Consent for Release of Information Form
• Verification of CMPA coverage
• Verification of CPSO membership
• OHIP number

If E.R. call is desired, applicant must also provide:

• Proof of current ACLS certification
• Proof of current ATLS certification
