Admitting
The Admitting Office is located in the front lobby. You will be asked to provide the following information: Where possible, your requests for semi-private or private accommodation will be met as availability permits. An additional charge for these types of accommodation is your responsibility. The hospital will submit the claims to your insurance; however, the responsibility for payment remains that of the patient. In some instances, prepayment may be required. 

CONSENTS 

You, or a member of your family, will be asked to complete and sign a consent form for admission and possibly a consent form for a specific procedure or treatment. 

The procedure to be performed will have been explained to you by your doctor. Before signing the consent, it is important that you understand what is to take place and what to expect as a result of the procedure. 

If there is anything you do not understand, please ask questions. Remember, it is your right to be fully informed. 

WHAT TO BRING WITH YOU 

To make your stay comfortable, you may bring your own nightgown, pajamas, robe, slippers and toiletries such as toothbrush and toothpaste, shampoo, etc... No electrical appliances, except razors and hair dryers, are permitted in the hospital. These are to be checked by a Building Services Team Member before use. Please do not bring jewelry or money except for small change, as the hospital cannot accept responsibility for loss or damage to personal articles left in your hospital room. 

All personal medications must be sent home, or left at the Nursing Station until your discharge. 

Patients are responsible for personal laundry.
